New therapeutic targets beyond amyloid-β will probably be needed to generate better therapeutic outcomes for patients with Alzheimer’s disease (AD). The transmembrane receptor TREM2 is emerging as one such target; it regulates key functions of microglia, enabling them to respond to pathological cues (including aggregated proteins). Previous efforts to activate TREM2 focused on antibody-based approaches to stabilize the membrane-bound protein, but several factors, including limited brain penetration, limited their utility — and early clinical studies failed to show benefits.
